Rates
$160 per 45-minute individual psychotherapy or relationship counseling session.

Self Pay
Out-of-pocket payment avoids insurance requirements and consequently offers better privacy. When insurance is used, the billing forms include diagnoses, service dates, and types of service.  Additionally, any insurance company can require clinical information when it decides to review for insurance eligibility. With self-pay, your information stays in my office. No information may be shared with any third party without obtaining your informed consent beforehand, unless information is required by the Court for a legal proceeding or there is a major threat to self or others. Those who choose self-pay will be furnished with a Good Faith Estimate of projected costs (See more on this subject below under Payment.).

Insurance
I am an in-network provider and accept reimbursement from Anthem BlueCross BlueShield, Humana Choice Care, and Medicare.  I shall bill them for you. Services may be covered at least in part by your health insurance or employee benefit plan. Please make sure of your coverage. You might want to check on your deductible and your copay.

At this time, the three companies cover telepsychology. Although telepsychology will continue to be covered for awhile, I don't know for how long. At this point, it seems that Medicare may continue to pay for it indefinitely. These companies do now and will continue to pay for in-person sessions. With self-pay, telepsychology can continue as long as needed.

Payment
Cash and checks are accepted for payment. 

If you use insurance and I am in-network, you will owe the copay or a percentage of the allowed fee, after having paid the deductible.  You will pay the deductible before insurance starts to pay its part directly to me.

If you don't have insurance or are not using your insurance, under the law you have the right to receive a "Good Faith Estimate" of the cost of your psychotherapy. Consequently you have the right to receive a Good Faith Estimate for the total expected cost of any services to be rendered that are reasonably anticipated under non-crisis circumstances. As soon as a course of treatment can be determined, make sure that you are given a Good Faith Estimate. Note that the general fee of $160 per session has been already declared above, although special circumstances may warrant its adjustment. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. It is suggested that you first bring up the issue with Dr. Fineberg for potential resolution. Make sure to save a copy or picture of your Good Faith Estimate.

Cancellation Policy
If you do not come to your scheduled therapy appointment and you have not given notification at least 24 hours in advance, you will be charged half my fee
 for the session. If you use insurance, know that insurance pays nothing for a missed appointment.
